2. By mailing or sending through courier service addressed to the above OMB Office where the applicant desires his/her clearance application processed.
3. By sending through electronic mail at the address provided above where the applicant desires his/her clearance application processed.
To facilitate the processing of the request, the applicant is advised to send with the application, the proof of payment (Postal Money Order check or receipt from the Landbank Link.BizPortal indicating the name of the applicant as the payor for easy matching with the applicant’s filled up and submitted application form), Service Record and government issued Identification Card with picture. The original Postal Money Order check should be received by the OMB Office where the applicant desires the application to be processed.
Applicants may pay through Postal Money Order payable to the “OFFICE OF THE OMBUDSMAN CLEARANCE FEES” and the Landbank Link.BizPortal.
