ONLINE FILING OF CLEARANCE

Please be advised that the SM payment facility is currently unavailable due to an on-going re-assessment by the Office of the Ombudsman and SM.

How to send your payment:

  1. Cash (temporarily unavailable due to the community quarantine)
  2. Landbank Link.BizPortal (click here to know more about sending payment via Landbank Link.BizPortal)
  3. Any SM branch (plus: service fee of P50.00 + private courier fee of P150.00) per copy (temporarily unavailable)
  4. Postal Money Order payable to “OFFICE OF THE OMBUDSMAN CLEARANCE FEES” directly to the concerned Ombudsman office:

Mode of Release:
Your clearance/certification will be due for release within reasonable time upon receipt of payment either:

  1. For pick-up (temporarily unavailable due to the community quarantine)
  2. By ordinary mail which is free of charge
  3. If you prefer to receive the clearance/certification earlier, you may send a prepaid envelope from any private courier

An offical receipt shall be attached to the duly issued Ombudsman Clearance or Certification.

INSTRUCTIONS

An application or request for issuance of Ombudsman Clearance may be filed at any time, however, for retirement purposes, an application shall be filed not earlier than six (6) months prior to the date of retirement of the applicant.

Please write N/A for each entry that does not apply to you.

Moreover, this online application is made under oath. Make sure that all information provided are true and correct to the best of your knowledge and is supported by proper and adequate documentation, and you must be prepared to produce the same when required in the future.

No Ombudsman clearance is necessary for the payment of the money value of the accrued leave credits (Terminal Leave Pay) of a separated official or employee, subject only to whatever claims the paying agency may have against the same (Per MC #5 Series of 2012, signed by Ombudsman Conchita Carpio Morales).

For retirement/resignation/separation purposes, please email your service record reflecting the date of retirement/resignation/separation to ombclearance@ombudsman.gov.ph

For death claim purposes, please email the service record and death certificate of the deceased to ombclearance@ombudsman.gov.ph
As additional requirement, please have a photocopy of the valid ID’s of the applicant and representative.

Effective July 01, 2019, first-time jobseekers availing free Ombudsman Clearance under Republic Act 11261, also known as the “First-Time Jobseekers Assistance Act” shall submit a barangay certification stating that the applicant is a first-time jobseeker; otherwise, he/she shall be liable under the Revised Penal Code if found guilty of fabrication or falsification. Such benefits provided under the said Act is not applicable for those applications made thru SM Business Center due to their System Restriction, and must proceed or send their application and payment directly to the nearest Ombudsman Offices.

This online facility caters only to applications filed individually and will not accept bulk applications.
