      Any person.

  • Request/grievance letter or duly accomplished Request for Assistance Form (OMB Form 2) (1 original copy)

  • For oath administration, valid ID (any of the following):

    1. Government-issued ID with picture

    2. Company-issued ID with picture

    3. School ID for students

    4. Integrated Bar of the Philippines ID (1 photocopy of the front and back of the ID to be compared with the original)


      Thirty (30) minutes
