effectivity date of retirement/resignation of the applicant and must be accompanied by a CERTIFIED
TRUE COPY OF THE LATEST/UPDATED SERVICE RECORD reflecting the date of retirement/resignation. |

documents such as Indorsement Letter, Latest/Updated Service Record and any valid ID with your picture and
signature. If for retirement or resignation purposes ensure that the date of retirement or resignation
is reflected in the service record duly signed by the Head of Personnel Division. |

adequate documentation, and must be prepared to produce the same when required in the future. |

No Ombudsman clearance is necessary for the payment of the money value of the accrued leave credits
(Terminal Leave Pay) of a separated official or employee, subject only to whatever claims the paying
agency may have against the same (Per MC #9 Series of 1994, dated August 22, 1994 signed by
Ombudsman Conrado M. Vasquez). |
